Role and Responsibilities:
Core Measures are a set of care standards, required by the Centers for Medicare & Medicaid Services (CMS), which describes the care to be provided to our patients while in the hospital. These care standards have been shown to improve patient outcomes. This individual will review the hospitals performance of patient care (Core Measures). Then will proceed to submitting a performance data to the applicable quality agencies in compliance with regulations. Which then enters data into computerized tracking system and analyzes data for trends. This position requires a sound knowledge of the medical record and team guidance abilities.
Qualifications and Education Requirements:
Graduate of approved accredited nursing program, RN/ BSN
Current license to practice as a Registered Professional Nurse
Use of the tools and techniques of continuously quality improvement and computer skills for data display.
Attention to detail and follow-up necessary.
Strong interpersonal skills, and the ability to communicate effectively with patients, families, hospital staff, physicians and community resources required.
Must be able to work independently.
